Important information about your membership.

Your Elite Care membership fee covers Dr. Liesa’s time and personal care. Lab testing, medications, immunizations, and special tests such as allergy testing, hormone testing and others, are additional.

Some tests and medications may be covered by your insurance. Dr. Liesa’s practice provides the information you need to submit to your insurance company for reimbursement paid directly to you.
